How CaseExchange Works
ProCaseo's CaseExchange service is based on the most up-to-date internet technology and its searchable database of CaseExchange network members.The network consiste of hospitals wishing to discharge patients to a continuing care environment in the most appropriate and efficient manner possible, together with the community of nursing homes, rehabilitation facilities and other services that can deliver that care.
CaseExchange was built specifically for this purpose and is backed up by ProCaseo's Customer Care organization, which supports both the database and the all those who use the system. Included are facilities and services that are not yet subscribers, providing the most comprehensive and transparent discharge management solution available today.
CaseExchange is primarily used by Discharge Planners and Admissions Coordinators, who access the web site using their standard web browser. No additional software is required by either party. In addition, extensive tools are provided for Discharge Planners to assist patients and families to locate and select their preferred care facilities. The service also provides immediate user notification of time-critical events, and a variety of management support reporting to systems administrators.
To initiate a transfer, a hospital Discharge Coordinator simply enters a patient's post-discharge requirements into the CaseExchange web site. These requirements are matched against the CaseExchange database of provider facility capabilities to produce a sorted and ranked listing of candidate destinations, with information about each facility. This is then reviewed with the patient and family to determine preferred destinations.
CaseExchange then creates Referrals to the selected destinations. These are typically received and reviewed by the facility’s Admissions Coordinators. Recognizing the time-critical and potentially competitive nature of these transactions, Admissions Coordinators can also be alerted by real-time notifications, such as a cell phone text message or e-mail.
While each potential destination facility is still required to evaluate the patient to determine whether they can adequately handle his or her requirements, CaseExchange facilitates and guides the collection of detailed patient data and supplies it along with the Referrals, vastly simplifying the process while improving communications and reducing the time required to complete the transfer.
CaseExchange helps to communicate Referral and Evaluation status, and protects patient information to ensure confidentiality and HIPAA compliance.
