ProCaseo is dedicated to supporting those who support patients as they move from one level of care to another. The basis of that support is the CaseExchange Network of facilities and services.

CaseExchange is a management and communication solution for hospitals, post-acute care facilities and organizations providing home-based healthcare to patients. If your organization fits one or more of these categories, you should already be a member.

The Network is built around two membership classifications:

  • Hospitals, which actively discharge patients. When used by Hospitals, Full Membership in the network entitles Discharge Planners to access the CaseExchange software to assist in placing patients.
  • Post-acute care facilities, which receive and admit patients for further care and/or rehabilitation. The Network supports three levels of membership for facilities providing post-hospitalization care. These are:

Full Membership. Facilities pay a subscription fee which allows them to list all of their capabilities, specialities and details about their services. These details are noted in the CaseExchange Software and match a patients needs against a destination's capabilities. CaseExchange then orders the results in a list provided to discharging case managers. Full members with admitting privileges get direct notification of referrals, access to the needs of the patient, and additional information to assist in the admissions decision.

Basic Membership. Post-acute facilities are listed by location, category of services offered, and contact information. Basic members may receive referrals from hospital members. This level of membership is designed to assist Discharge Planners with those unusual and out-of area placements that can take so much time specifically because there is not an existing relationship and regular communication between the parties.

Associate Membership. For facilities and/organizations that regularly work with one of our full hospital members, you automatically become an Associate Member. This allows our Customer Care organization to work with you to list your availability to accept referrals. Our full members need this information to locate the best possible destinations for their patients. This level of membership is limited.
